SCHEDULING

When scheduling there will be a series of questions. Although they are not required, they will save us time at your appointment. As the saying goes, “Time is money!”

You must check the terms and conditions and HIPAA boxes to schedule. Please note: NO nursing documentation will be added on my webpage site, other than what you put in for scheduling. My charting will be kept on private personal files and not accessible online.

If you own pets please contain them prior to your appointment time. I LOVE animals, but prefer a proper introduction. Please let me know what to expect.
 

 

My primary interest in nursing has been in hospitals. My 28 years of experience includes working psychiatry, medical/surgical, adjunct clinical instructor, progressive care/step down, and ICU. Almost all of my roles have included precepting student and new graduates. I traveled as an independent contractor for several years in Wyoming, Idaho and Texas. Basic life support and advanced cardiac life support certified. My own personal experience with the challenges of the healthcare system, has lead to starting Heartfelt Services.

TERMS AND CONDITIONS

General: Heartfelt Services, LLC is a fee for service nursing business and retains the right to refuse service without cause. We will accomplish as much as humanly possible during your service time. Some services, such as the MLOA/STD assisting with paperwork takes time. The primary holdup will be obtaining physician signatures after submitting the required paperwork. Heartfelt Service, LLC has no control over the priorities of others and cannot be held liable for delays nor additional time requirements as a result of others. The advocacy service will also take time. Our goal is to assist you in preparing for your visits with healthcare providers to streamline the delays and expense. We will advocate for you when needed to assure you are getting the attention you deserve for the cost of your visit. We will provide education, motivation, and nursing within our scope of practice. Orders from your physician will be required for some services.

 

Insurance: At this time we do not file insurance claims nor accept  Medicaid nor Medicare. Invoices can be provided for your records.

 

Rates: All  services are set up on an hourly rate, except for smoking cessation classes.  In the event you are unhappy with us, please speak to us directly. We expect to get paid for our time, but we all know that healthcare is not without its perils. Nursing does not always get the desired outcomes, but we put in the work regardless.

 

Third-party care: In order to evaluate the care of loved one in a facility you must be a direct family member, be an emergency contact, or have power of attorney. Your loved one will need to be in agreement to have our services as well.  If they are confused or unable to give consent, you must have power of attorney and provide us with a copy. No exceptions.

 

Smoking Cessation Classes: The smoking cessation classes will be held in a small group setting, 1 hour a week for 6 weeks. Payment is required in advance for the classes. The location of the classes will be announced before sign up. Neither Heartfelt Services, LLC nor the location of the meetings will be held responsible for any accidents. 

 

Cancellations: We know that things come up in life, so if you need to cancel please provide a 24 hour notice. If  you are unable to do so, message us! We will refund a less than 24 hour cancellation once. However, if we make a trip to your location and you are not present at your appointed time, you will not be refunded. After the one-time refund, you will be charged the appropriate service rate if you provide a less than 24 hour cancellation.
